Oregon Drug Rehab Treatment Provider Database

Oregon continues to experience high rates of substance use, with fentanyl, methamphetamine, and alcohol posing major challenges for both urban and rural communities. From Portland and Eugene to small towns across the state, many residents face barriers to timely care. Through the Oregon Health Plan (OHP), which is Oregon’s Medicaid program, eligible individuals can access coverage for detox, residential, and outpatient treatment. This searchable database makes it easier for Oregonians to connect with licensed providers and find the help they need close to home.

How to Use the Database

  1. Enter a 5-digit Oregon ZIP code in the “ZIP Code” box. (No need to press Enter.)
  2. Select a search radius from the “Select Range” dropdown menu.
  3. Choose one option from each of the three filter categories. You can select them in any order, but all three filters must be selected to proceed.
  4. After selecting the ZIP code, range, and all filters, click the “Search” button to display a list of matching programs.

  • Limited Results for Specific Filters: Filters such as males only, females only, adolescents, residential, telehealth, and detox may return a limited number of facilities. If your initial search yields no results, click the back button in the upper left corner of the results screen and try increasing the search radius.
  • Opioid Misuse Filter: The opioid misuse filter lists facilities that exclusively treat opioid dependency. However, other facilities may offer opioid misuse services.
  • Level of Care: If you’re uncertain about the level of care required, search for an outpatient clinic near you and call them to schedule an assessment.
  • Call Ahead: To avoid confusion, it's suggested to phone a facility first, before visiting.
